This is a simplified risk diagram for a syringe pump based opiod delivery system. The size of the circle cooresponds to the probability and the color cooresponds to the severity. This diagram shows that the risk for the device is fairly high, and if you clikc on the circles to show a further breakdown you can see that the single largest contributor to the risk is the 'Patient intentionally changes rate higher'. If you hover over that circle, the mitigations are shown - in this case only a 4 digit lock out code. For an addictive pain killer that mitigation is not very effective, hence the high risk of over infusion.
